Pre-K Income Verification Form.

Please complete all the highlighted sections and attach proof of income to the form.

School Entrance Health Physical Form.

Please complete the front of the school health physical form and take to the doctor for the physical exam portion to be completed.

Pre-K Registration Survey.

Please complete this survey using the Code: abc123 for all Pre-K students.

Proof of Residency Form.

This form needs to be completed for all students registering for Pre-K and residency documentation attached to the form.

Certification of Residency for Those with No Utilities in Parent/Guardian Name.

This form needs to be completed by all students registering for Pre-K if utilities are not in the name of the parent/guardian.  This form needs to be notarized at the Central Office.
